Advantage of Tele-Health
Using technology to deliver health care has several advantages, including cost savings, convenience, and the ability to provide care to people with mobility limitations, or those in rural areas who don’t have access to a local doctor or clinic.

Benefits for patients

  • Lower Costs: Some research suggests that people who use telemedicine spend less time in the hospital, providing cost savings. Also, less commuting time may mean fewer secondary expenses, such as childcare and gas.
  • Convenience: Telemedicine allows people to access care in the comfort and privacy of their own homes. This may mean that a person does not have to take time off of work or arrange for childcare.
  • Slow the spread of infection: Going to the doctor’s office means being around people who may be sick, often in close quarters. This can be particularly dangerous for people with underlying conditions or weak immune systems. Telemedicine eliminates the risk of picking up an infection at the doctor’s office.
  • Preventive care: Telemedicine may make it easier for people to access preventive care that improves their long-term health. This is especially true for people with financial or geographic barriers to quality care.

House-call Visits:  Wellness, Sick visits, and Monthly follow-up visits for homebound patients in the comfort of their homes. House-call services are under “Higher Ground House Calls”

We deliver expert medical care to homebound patients and those with complex problems. During house call visits, we analyze history, perform examinations, order labs, diagnose, and prescribe treatments appropriate for individual patients’ needs. Moreover, when it comes to chronic disease management like diabetes, hypertension, osteoarthritis, etc. we educate patients on self-management plans and assess patient condition, set reminders for timely medicine intake, and also spend time to teach and educate the patient, and their families on disease process and treatment plans.

The main benefit of in-home medical care for the elderly is that it helps keep patients out of hospitals, emergency rooms, and nursing homes.

In order to qualify for in-home doctor visits, there are a wide variety of potential reasons, but some of the most common include the following:

  • Mobility problems such as disability or injury
  • Lack of transportation (or access to transportation)
  • Vision problems that make driving or traveling dangerous
  • Mental health problems or memory problems (such as Alzheimer’s)
  • Multiple chronic conditions with complex management

Homecare Services:  In-home personal attendant services for individuals who qualify.

Provided to the elderly; people with ongoing physical or mental disabilities.

Attendants help individuals with activities of daily living, such as bathing, grooming, meal preparation, and housekeeping.

BP Remote Monitoring:  We will remotely monitor your blood pressure and assist you with education and counseling. Between sessions, remote blood pressure monitoring enables clinicians to closely monitor hypertensive patients. Remote patient monitoring can assist in preventing the development of chronic diseases and lowering healthcare expenditures by putting an emphasis on early detection and treatment.

The benefits of the Tele monitoring program for patients include

  • Decreasing hospital re-admissions
  • Decreasing unnecessary emergency room visits
  • Decreasing travel expenses, and time
  • Decreasing time away from work.
  • Decreasing healthcare cost
  • Improving management and monitoring of chronic Medical conditions
  • Improving overall patient health and outcomes
  • Improving constant communication between patients and physicians

Annual Wellness Visits:  We will help with your annual physical exams and your immunization requirements.  The journey toward bettering a member’s health can begin with a solid working relationship with a dependable provider and a satisfying interaction with the health plan.

Once these links are made, it is simpler to determine the actions that members should take to manage pre-existing conditions, obtain the preventative screenings they require to spot emerging problems early and gain a better understanding of how to care for themselves.

Additionally, these visits are necessary for practitioners working to achieve community health objectives, which begin with a risk and health assessment. The AWV is a crucial component of health plans’ efforts to manage care effectively and improve long-term outcomes while lowering costs for all parties involved.

Chronic Disease Management:  Management of acute and chronic diagnosis, disease prevention, education, and counseling on health maintenance. Additionally, when it comes to managing chronic diseases like diabetes, hypertension, osteoarthritis, etc. We educate patients on self-management plans, evaluate patient health, set reminders for timely medication intake, and spend time educating the patient and their families on the disease process and treatment options.
